
The Breath Serves As the Connection Between The Body And Mind, And Box Breathing Is One Of The Easiest Techniques To Bridge That Gap.
If you've spent any time in a yoga class, gone through a Navy SEAL interview, or used a wellness app, you may have encountered box breathing. This technique is simple, free, and takes less than five minutes. These qualities contribute to its popularity as one of the most searched breathing techniques online. But does it actually work, and how does it compare to other breathing methods? Here’s what the evidence says.
Box breathing, also known as square breathing or 4-4-4-4 breathing, is a slow and controlled breathing pattern consisting of four equal phases:
1. Inhale for 4 seconds
2. Hold the breath for 4 seconds
3. Exhale for 4 seconds
4. Hold the breath for 4 seconds
Each phase corresponds to one “side” of an imaginary square, which is where the name comes from. This technique is a form of diaphragmatic (or belly) breathing and falls under the category of “paced” breathing. It involves deliberately slowing your breathing rate to influence your nervous system.
Your breathing rate is closely connected to your autonomic nervous system, which regulates your fight-or-flight (sympathetic) and rest-and-digest (parasympathetic) responses. When you breathe quickly and deep, it indicates to your body that there is a threat. In contrast, slowing your breath helps prompt your body into a calmer state.
A review published in 2025 on slow, controlled breathing practices found consistent associations with improved vagal tone, greater heart rate variability, and increased parasympathetic activity. Additionally, it showed reductions in cortisol levels, anxiety, and stress. Furthermore, research using EEG on slow breathing revealed that it enhances alpha and theta brain-wave activity in areas linked to calm and focused mental states.

It’s important to clarify that much of the strongest evidence for anxiety reduction in this context comes from breathwork in general rather than specifically from box breathing. This distinction is important for how you choose to apply it.
This is the part that many articles overlook, yet it is the most valuable information for those trying box breathing specifically to manage anxiety.
A well-cited study from Stanford compared box breathing with two other techniques: cyclic sighing (a double inhale followed by a long, slow exhale) and cyclic hyperventilation. Participants practiced each method for five minutes daily over the course of a month. All three groups showed improvement; however, the researchers hypothesized that techniques emphasizing a longer exhale would be more effective for reducing anxiety. This is because box breathing maintains equal lengths for all phases rather than favoring the exhale.
Additionally, a separate analysis from 2023 found that daily breathwork, which included box breathing, significantly reduced anxiety and improved mood more than an equivalent duration of mindfulness meditation. However, this analysis did not isolate the specific contribution of box breathing.

A narrative review from 2025 on breathwork protocols reached a similar conclusion: box breathing showed a weaker alignment with the vagal-tone-boosting mechanisms associated with slower, exhale-focused methods. However, it was noted that controlled breathing, when practiced consistently, was linked to improved stress outcomes overall.
You don’t need any equipment, and you can do this exercise while sitting, standing, or lying down.
1. Sit comfortably with your back supported and your shoulders relaxed. Close your eyes or soften your gaze.
2. Exhale fully through your mouth to empty your lungs.
3. Inhale slowly through your nose for 4 seconds, allowing your belly to expand.
4. Hold your breath for 4 seconds—stay relaxed and avoid clenching.
5. Exhale slowly through your mouth for 4 seconds, completely emptying your lungs.
6. Hold your breath at the bottom for 4 seconds before starting the next cycle.
Repeat this process for 4–5 minutes or for 4–8 cycles to start. If the 4-second intervals feel strenuous, especially during the holds, begin with a 3-3-3-3 count and gradually build up. The goal is to achieve calm control, not to focus on breath-holding endurance.

Researchers have also started to explore this technique clinically. A randomized trial scheduled for 2025–2026 will investigate the effects of box breathing on anxiety, pain, and nausea in patients undergoing an endoscopic procedure (ERCP). This reflects a growing interest in its use as a cost-effective adjunct in medical care.
